Notice to Texas Consumers
In accordance with House Bill 4224 (89th Regular Session) effective September 1, 2025 and Section 181.105 of the Texas Health and Safety Code, Texas-licensed behavioral health providers are required to make the following consumer information publicly available.
Consumers have the right to:
Request access to their healthcare records from the licensee
File a Consumer Complaint with the Office of the Attorney General’s Consumer Protection Division
Contact the Texas Behavioral Health Executive Council (BHEC)

Consumers may request access to their health care records maintained by Preston Hollow Counseling, PLLC. To request health care records:
E-mail a written request to abby@prestonhollowcounseling.com
While processing requests, Preston Hollow Counseling will follow all applicable state and federal laws.
A Release of Information (ROI) for must be completed and requests may take up to 30 days.
Fees may apply as permitted by law.
Please note that under certain limited circumstances as listed in Texas Health and Safety Code Chapter 611.0045, access may be denied to all or portions of the private health information (PHI) requested.

Texas consumers may file a complaint against a business by contacting the Office of the Texas Attorney General.

Consumers may contact the Texas Behavioral Health Executive Council regarding Texas-licensed behavioral health providers.
The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology. Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint.
